In order to set up emergency 911 service on your phone line, you will need to fill out the supplied form.

Why is This Document Required?

Unlike analog phone lines that are hard-wired into your city’s network, voice over internet phones rely on an internet connection and do not connect to your city’s infrastructure. Therefore, city services like 911 do not work if not properly set up. This form will allow your local city to automatically recognize calls made to 911 from your phone number to auto-populate your address when calling.
